Contemporary hospital design aims to maximise patients' comfort and satisfaction by implementing a patientcentric design. However, systematic analyses of the social dynamics of patient-caregiver relationships are still limited. To fill this research gap, this paper develops an agent simulation model that computes spatial relationships between patients and caregivers and perceives interpersonal behaviours via agents' vision sensors. This paper compares the results of utilisation analyses performed using this simulation model while integrating patient-caregiver interpersonal behaviours with results from a model that neglects these interactions across six hospitals and assesses the validity and reliability of the simulation results (N = 36). A statistical analysis reveals that the social dynamics simulation improves the reliability and validity of hospital utilisation analysis including congestion, delay time, patient-caregiver connectivity and interference with personal space. This is because the simulation model facilitates interpersonal interactions between patient and caregiver groups in a manner that accurately reflects real-life behaviour.
