Backgrounds: Vasovagal syncope(VVS) is common cause of recurrent syncope. But the exact hemodynamic mechanism has not been elucidated. The pulse wave analysis(PWA) is widely used to evaluate vascular property which reflects the condition of the entire arterial system. Aims: To investigate the association between findings of pulse wave analysis(PWA) in VVS patients. Methods: We investigated consecutive patients who suspected VVS and performed head-up tilt test(HUT) and PWA from June 2014 to May 2017. The criteria of the heart rate-corrected augmentation index(AIx@75) and pulse-wave velocity(PWV) was estimated as a composite marker of wave reflections and arterial stiffness. Results: A total of 72 VVS patients were enrolled (mean age¼40618, male¼23). Compared to the age and sex matched healthy subject group (N¼72, mean age¼40617, male¼26), the VVS patients had the higher AIx@75 value than control group(18.4612.4 vs. 12.5614.2, P<0.01) and also ahd higher pulse wave velocity(PWV) than control goup(6.461.4 vs. 5.861.0, P¼0.01). In subgroup analysis( age<40 years), AIx@75 showed significantly higher in VVS than control group and tends to be more higher in young VVS patients(P<0.001). Conclusion: This study suggest that VVS patients were increased arterial stiffness and especially young VVS patients tend to further increased AIx@75 .
