A 84-year-old man was transfered from the private clinic due to severe abdominal pain and detected intraabdominal free-gas on chest X-ray. In exploration, the stomach and duodenum from 3rd portion were severely distened, and the small perforative lesions were seen due to serosal ishcemia on the proximal portion of stomach. The total gastrectomy with Roux-en-Y anastomosis and the releasing of Treit's ligament were done.
