Effect on Intermittent Pneumatic Compression and Graduated Compression Stocking on Deep Vein Thrombosis Prophylaxis

Purpose. Deep vein thrombosis (DVT) can cause fetal pulmonary embolism. Critically ill surgery patients are one of the high risk groups. Even if the incidence of DVT among Asian was lower than those of western studies, DVT should be prevented because of potential fatality. The purpose of this study was to investigate the effect of mechanical interventions for deep vein thrombosis prophylaxis in SICU patients. Methods. This study employed a quasi-experimental design. The data were collected at a university hospital. The subjects were 165 SICU patients who were not received any prophylactic medication during the study period. However, data for 147 subjects were included in final analysis due to drop out. The subjects were assigned to one of two experimental groups; intermittent pneumatic compression (IPC) intervention or graduated compression stocking (GCS) intervention. Control subjects who met the criteria were selected from previous study for comparison. Results. Mean age was 58.04 (±15.9) and 60.9 percent of them were male. Sixty-three percent of them were over 61 years old. In second duplex scan, patients who developed DVT were 4(7.3&%), 2(5.0%), and 1(1.9%) respectively for control, GCS, and IPC group. The mean risks core was 7.24(±2.47), if it was calculated by Capri (2005)’s formulation. Most frequently reported factor was mechanical ventilation therapy in ‘1 point items’ and 104 patients received mechanical ventilation therapy. The second most frequent risk factors were age and bed rest status. Conclusion. Although this investigation had limitation of small sample size and no random assignment, the author could conclude that mechanical prophylaxis is effective prevention of DVT among SICU patients.
