Aldosterone plays a critical role in the pathophysiology of heart failure (HF), contributing to adverse cardiac remodeling, fluid retention, and increased systemic vascular resistance. Specifically, excessive aldosterone activates inflammatory responses and oxidative stress pathways, which drive myocardial fibrosis and left ventricular hypertrophy. Targeting aldosterone synthesis has thus become an important therapeutic goal for improving patient outcomes in HF, particularly within the context of mineralocorticoid receptor (MR) antagonism. Aldosterone synthase inhibitors represent a novel approach to heart failure management. By specifically blocking aldosterone production at its source, these inhibitors reduce systemic and local detrimental effects on the myocardium and vasculature. Unlike traditional MR antagonists, which competitively block the receptor, aldosterone synthase inhibitors act upstream in the biosynthesis pathway, potentially minimizing hyperkalemia and other limitations observed with MR antagonists. For instance, clinical candidates such as LCI699 have shown promise in preclinical and early-phase trials by effectively lowering aldosterone levels without directly impacting potassium homeostasis. This presentation will cover the mechanism of action of aldosterone synthase inhibitors, their potential benefits over current therapies, and recent findings from clinical trials. Emphasis will be placed on their role in reducing cardiac fibrosis, improving diastolic function, and mitigating inflammation in heart failure patients. We will also explore how this class of drugs fits into the current treatment landscape and their potential to provide additive benefits when combined with standard heart failure therapies.
