We were experienced a case of spindle cell liposarcoma in left posterior neck of a 70 year-old man. On physical examination, the tumor was movable and measured 10.0X10.0cm. Computed tomography revealed a soft tissue tumor in inner portion of trapezius muscle on left posterior neck space. Grossly the tumor was an ovoid-shaped soft mass with irregular margin and measured 5.0x3.0x3.0cm. The cut surface is yellowish gray and myxoid. Microscopically, the tumor showed hypocellular myxoid or hyaline areas alternating with areas showing greater cellularity. The tumor mainly consisted of spindle cells arranged in short fascicles ir whorls with patternless, moderately rich capillary network. The spindle cells had slightly enlarged hyperchromatic nuclei and poorly demarcated, faintly eosinophilic cytoplasm. The intercellular matrix showed myxoid change. The tumor also showed an adipocytic component. Mitotic figures were scarce. Immunohistochemically, spindle cells were positive for vimentin, desmin and CD34 and negative for smooth muscle actin. The atypical adipocytic cells were positive for S-100 protein.
