Cushing’s syndrome associated with nodular adrenal hyperplasia glands is divided into 4 main categories: adrenal adenoma, adrenal carcinoma, primary pigmented nodular adrenal hyperplasia and macronodular adrenal hyperplasia (MAH). The mechanism of bilateral MAH, when ACTH is suppressed, was previous unknown and was referred to as being “autonomous”. Recently, several reports have shown MAH are under the control of ectopic membrane hormone. These may include ectopic hormone receptors, such as those for GIP, β-adrenergic agonists, LH/hCG, or other receptors capable of coupling to G proteins, AC, and steroidogenesis. We report a case of Cushing’s syndrome caused by bilateral macronodular hyperplasia (MAH) and presence of aberrant recptor(s).
