Clinical manifestation and neuroimaging findings

Recently neuroimaging has allowed medical teams to find the clinico- anatomic collerated lesions in the patients with stroke. A knowledge of cerebral artery and supplied territories is need to decide the stroke etiology and treatment options. Brain consists of cerebral hemisphere, brain stem and cerebellum. Major cerebral arterial system includes internal carotid arteries and vertebral arteries. Bilateral internal carotid arteries supplied the anterior circulation, including cerebral hemisphere. Bilateral vertebral arteries supplied the posterier circulation like cerebellum, brain stem and posterior part of hemisphere. Also cerebral arterial circulation is diveded into two systems: the leptomeningeal arteries and the perforating arteries. Cerebellar lesions produce ataxia and dizziness. Brain stem infarct can makes the cross sign and Hornor' sign, due to involvement of nucleus of cranial nerve and ANS in brain stem. The patients with lesions in deep structure of cerebral hemisphere might have pure motor stroke, pure sensory stroke and sensorimotor stroke. The cortical signs like aphasia and neglect is related to the relevant cortial area. This lecture is designed to show precise cerebral arterial circulation organization and the clinico- anatomical correlations.
