Analysis of Results of Transurethral Resection of the Prostate Based on the Resection Ratio of in Patients with Large Prostates

Purpose: We determined the post - transurethral resection of the prostate (TURP) results based on the transitional zone resection ratio in patients with large prostates (>80gm) Materials and Methods: Between september 2005 and December 2007, 31 patients with symptomatic benign prostatic hyperplasia (BPH) which were larger than 80gm had undergone TURP were enrolled in this study. The evaluation before TURP included the International Prostate Symptom Score (IPSS), maximum urinary flow rate (Qmax), post-void residual urine (PVR), and transrectal ultrasonography. The operative time, weight of resected tissue, change in serum hemoglobin, and complications were noted. After TURP, patients were reassessed for the IPSS, Qmax, and PVR at 6 months. The analysis of the postoperative symptom scores was performed based on the ratio of the resection volume (RV) to the transitional zone volume (TZV). Results : The mean total prostate volume and mean TZV were 108.1±26.4gm and 66.9±19.8gm, respectively. The resected prostate volumes was 40.0±17.2gm. The mean ratio of the resection volume (RV) to TZV was 40.0±17 % . The postoperative IPSS, PVR and maximal flow rate (Qmax) were significantly improved. Severe bleeding necessitated blood transfusion, and the TURP syndrome did not occur in any patients. Conclusions: TURP is an effective and safe surgical treatment method in patients with large prostates. The symptom score after TURP improved more as the RV/TZV increased. For good results in TURP with large prostate , the resection volume ratio of prostate to TZV must be more than 70%.
