Firtst choice of shunting or cranioplasty in posttraumatic hydrocephalus with large flaccid cranial defect

Objective : The cranioplasty and ventriculoperitoneal (VP) shunt operation have been used to treat posttraumatic hydrocephalus with large cranial defect. The aim of this study was to compare outcomes between the patients cranioplasty the first (group 1) and the cranioplasty after the shunting (group 2). Methods : From 2002 to 2013, medical records and radiographic findings of 39 patients who undergone cranioplasty and VP shunt operation after the decompressive craniectomy were analyzed. The outcomes after operations were compared in two groups 6 months Results : The improvement of Glasgow outcome scale (GOS) was seen in 13 cases (68.4%) of total 19 cases in group 1, and 8 cases (40.0%) of 20 cases in group 2. Five (71.4%) of 7 cases with hemiparesis in group 1 and 2 of 11 cases (18.2%) in group 2 were improved. Conclusion : We conclude that the outcomes of shunt operation after cranioplasty may be better than cranioplasty after shunt operation in posttraumatic hydrocephalus with large cranial defect.
