Background : The prevalence of gout is on the rise, making urate-lowering therapy (ULT) an essential component of its management. However, adherence to ULT is reported to be low, highlighting the need to investigate the factors that influence non-adherence. Methods : This study analyzed data from 792 patients diagnosed with gout who were newly prescribed ULT at a university hospital in Incheon in 2017. The dataset included demographic information, medication usage details, and prescription specifics. Medication adherence was assessed using the Medication Pos session Ratio (MPR) over a 365-day period. The complexity of medication regimens was evaluated using the Korean Medication Regimen Complexity Index (MRCI-K). Factors associated with non-adherence were analyzed using multinomial logistic regression, comparing groups with serum uric acid levels above and below 6 mg/dL. Results : The majority of patients were male, with a median age of 52 years. The median number of med ications, excluding ULT, was five, and the median prescription complexity score was 18. The adherence rate was found to be 54.4%. Patients with higher serum uric acid levels showed significant differences in age, regimen complexity, and MPR. Logistic regression revealed that younger age, higher uric acid lev els, greater regimen complexity, and lower Charlson Comorbidity Index (CCI) scores were associated with non-adherence. Conclusion : The medication adherence rate among gout patients was as low as 54.4%, significantly correlating with serum uric acid levels, regimen complexity, and age. Targeted strategies and interven tions are needed to enhance adherence and improve patient outcomes.
