Primary papillary carcinoma arising in a thyroglossal duct cyst

The thyroglossal duct cyst is reperted to persist in 7% of adult, and a thyroglossal duct cyst is the most common disease among congenital mass of the neck. About 1% of thyroglossal duct cyst are histologially malignant, and 80% of malignant case are those of papillary carcinoma of the thyroid. A 31-year-old female with papillary carcinoma of the thyroid origating in a thyroglossal duct cyst is described. The patient's chief complaint was anterior neck mass. Ct examination shows mltiseptated, large cystic mass with calcification and enhancng solid area in part of the interior of the mass. Resection of the mass atempted with a diagnosis of the thyrogolssal duct cyst. The mass was multinodular cystic mass, measuring 6.0x5.5cm with a segment of hyoid bone. On section, there was a multilobulated cyst which is filled with yellow mucinous fluid. The inner surface showed a few papilary projection, measuring 0.8cm in diameter of the largest one. The remaining surface is grayis brown, glistening, trabeculaed and sclerotic. Since papillary carcinoma of the thyroid was noted in the cyst by pathologic examination, primary carcinoma of the thyroid was susped. Total thyroidectomy and neck lymph node dissection was performed. The thyroid gland revealed nocular hyperplasia and no tumor. The resected neck lymph node revealed metastatic papillary carcinoma. So the tumor was considered to be primary papillary carcinoma of the thyroid arising in the thyroglossal duct cyst.
