Background: Basal cell carcinoma (BCC) of the prostate is rare tumor and its biologic behavior is not well known because rarity of tumors with short follow-up. Case Report: A 74-year-old man came to the hospital with the impression of prostate tumor. He had urinary obstruction symptoms and underwent trans-urethral resection of prostate at the local clinic under the impression of benign prostatic hyperplasia. Serum PSA level was within normal range (0.65 ng/mL) at the operation time. The prostate chips revealed irregularly distributed small basal cell clusters or tubules with peripheral palisading. Some clusters showed cribriform pattern with eosinophilic basement membrane material or mucinous basophilic secretions. Tumor clusters showed infiltrative pattern. Tumor cells were positive for bcl2 and Ki67 index was about 20%. Conclusions: The infiltrative pattern, bcl2 positivity and high Ki67 index in the present case can be useful findings to differentiate this tumor from benign prostate hyperplasia.
