Comparison of the complication on donor and recipient between suprafascial and subfascial radial forearm free flap

Purpose: Pleomorphic adenoma is a benign tumor that can occur in the salivary glands. It is most commonly found in the parotid gland or submandibular gland. But rarely, it can also occur in the minor salivary glands. These glands are located throughout the mouth, including the upper lip. We would like to share the experience of a rare case for pleomorphic adenoma on upper lip. Methods: From 2006 to 2021, we collected 32 patients who underwent RFFF for head and neck reconstruction. All flaps were performed by one senior surgeon. Patients were classified into two groups according to the flap elevation method such as subfascial dissection (Group A) and suprafascial dissection (Group B). Data were collected with medical record such as, patient information, flap size, donor site and recipient site complication. Differences and group characteristics were compared. In addition, the data of our institution was compared with the reported literature. Results: There were total 32 patients underwent RFFF, 13 patients in Group A, 19 patients in Group B. The mean age was 56.15 years and there were 10 males (77%) and 3 females (23%) in Group A. And in Group B, the mean age was 59.11 years and there were 16 males (84%), 3 females (16%). All patients had head and neck reconstructive surgery. Mean follow up period was 8months in both groups. In Group A, mean area of defect was 42.83cm², mean flap size was from 50.96cm² and ADM(Allodermⓡ and Suredermⓡ) was used at donor sites in 3 patients. In Group B, mean area of defect was 33.33cm², mean flap size was 44.54cm² and ADM(Matridermⓡ) was used at donor sites in 17 patients (90%). Total 13 patients had donor site complications, 8 (62%) in Group A and 5 (26%) in Group B. The incidence rate of secondary surgery due tendon exposure was 23% in Group A and 0% in Group B. (p<0.05) All flaps were completely survival. Recipient site complication was 2 patient (15%) in Group A and 3 patients (16%) in Group B.
