방사선학적 척추 성상의 변화와 expandable cages를 이용한 PLIF 후의 성공적인 결과에 영향을 미치는 중요한 인자들

초록

Object: To analysis a minimal follow-up data of one year and to determine the factors influencing the successful results in the patients were performed lumbar arthrodesis with expandable cage(VariliftTM) at L34 to L5S1 intervertebral space, medical records and radiological data were inspected. Materials and methods: The authors included only thirty-five patients who could be confirmed bony fusion in the 12 month after a posterior interbody fusion procedure in which a one-level expandable cage was used. All patients were classified into group A(satisfactory results) and group B (unsatisfactory results) according as their answer to comprehensive low back questionnaires. The changes of radiological profile including the intervertebral disc height, segmental lordosis and total lumbar lordosis in the standing neutral lateral radiographs before/after surgery were compared. Results: 17 cases(48.6%) of "excellent" results, 14 cases(40.0%) of "good " results, and were included in Group A. In group B, 2 cases (5.7%) of " fair" results and 2 cases(5.7%) of "poor" results were included. Twelve months after operation, the mean intervertebral disc height of group A and segmental lordotic angle was significantly increased, but that in group B was not significantly increased; however, the improvement of lumbar lordotic angle could not observed on the both group. Conclusion: This results indicated that the recovery of intervertebral disc height, segmental lordosis accompanied with solid arthrodesis should be need to successful results and a significant influence of one-level segmental lordotic improvement on the total lumbar lordosis was not observed after expandable cage fusion in the lower lumbar spine.
