A Regression Analysis of Clinical Dementia Rating scale and Global Deterioration Scale

Background: Clinical Dementia Rating Scale (CDR) and Global Deterioration Scale (GDS) are commonly used global measures of dementia. However, it is difficult to translate scores of CDR into those of GDS and vice versa , because litte data comparing these two scales have been published yet. Objective: To calculate a regression formula allowing a comparison of cross-sectional data from two global measures dementia. Methods: Thirty-two non-demented controls, 37 patients with Alzheimer’s disease, 39 with vascular dementia and 2 with mixed dementia participated in our study. One of the two raters interviewed the subject in the presence of his or her caregiver and rated CDR (global CDR, Sum of boxes), and the other rater observed the interview and assessed GDS. The two raters assessed CDR and GDS independently and alternatively. The interview comprised questions for cognition, behavior, activities of daily living and part (orientation, registration, calculation and recall) of the Korean version of Mini-Mental State Examination (K-MMSE). Results of CDR and GDS were compared with those of the K-MMSE, Korean version of Syndrom-Kurz Test (K-SKT) and 15 items abbreviated Korean version of Boston Naming Test (SK-BNT). Results: Each of CDR, CDR-SB and GDS correlated significantly with neuropsychological measures (K-MMSE, K-SKT and K-BNT). Significant slopes in the regression occurred between GDS and CDR-SB (rs=0.79, slope=4.19, intercept=--8.76, p<0.001) and between GDS and CDR (rs =0.79, slope=0.70, intercept=--1.39, p<0.001). Conclusion: Our results allow translation of CDR or CDR-SB scores into GDS scores, and vice versa.
