Atalantoaxial subluxation in psoriatic arthritis

Object: Atlantoaxial subluxation associated with psoriatic arthritis is rare, with only a fwe case reported. The clinical findings and treatment results of a case will be presented. Material: We present a successful managed case with A 32-year-old man presented with a 10-year history of psoriatic arthritis, and more recently aggravated neckpain. Result: The patient was previouly managed with atlantoaxial wiring procedure at 10 years ago, but recently neckpain was developed without neurological deficit. Cervical dynamic spine x-ray demonstrated a significant atlantoaxial instability. Neckpain was resolved with transarticular screw fixation on atlantoaxial area and postoperative dynamic spine x-ray was not showed a instability. Discussion: The most often used surgical procedure for treating psoriatic arthritis is the same as that for rheumatoid arthrits: posterior craniocervical or atlantoaxial fusion with or without decompressive laminectomy. However, the authors is not enough to apply wiring fixation on psoriatic atlantoaxial subluxation and recommend the transarticular screw fixation like rheumatoid arthritis.
