Is dehydration test using isosorbide useful in Meniere’s disease? : Preliminary results of a multicenter prospective study

목적: Although dehydration test are relatively specific for endolymphatic hydrops, the test using glycerol is associated with several unpleasant side effects. This study is to examine the usefulness of the dehydration test using oral isosorbide that have been used as treatment for Meniere’s disease(MD) 방법:Electrocochleography(ECoG) and the dehydration test using isosorbide were performed in 32 patients with definite MD. After baseline audiometric test, isosorbide(120cc) was administered. Repeat audiometric test were performed at 3 hours after the isosorbide ingestion. Abnormal finding of ECoG was defined as SP/AP ratio ≥ 0.4. When there is a 10 dB or more improvement at two or more frequencies or there is a 12 percent or greater improvement in speech discrimination scores, dehydration test was considered positive 결과:The abnormal findings in ECoG were noted in 14 of 32 (43.8%). The positive findings in dehydration test using isosorbide were noted in 10 of 32(31.3%). Serious side effects were not detected after the test. When we defined endolymphatic hydrops as abnormal ECoG or positive dehydration test, it was detected in 21 of 32(65.6%). Although not statistically significant, the positive ratio of dehydration test in late stage of MD(stage 3 and 4) was higher than early stage of MD(stage 1 and 2) 결론:Dehydration test using isosorbide is a tolerable test that can detect endolymphatic hydrops and it can be used additionally as in an attempt to improve the sensitivity of ECoG in MD.
