The effectiveness of dermoscopy in non-melanoma skin cancer(NMSC):Decrease of the final number of Mohs microscopic surgery(MMS) Stages

Background: Dermoscopy has become an important equipment of the non-melanoma skin cancer(NMSC). In dermoscopic patterns, basal cell carcinoma(BCC) shows microarborizing telangiectasias and irregularly shaped globular structures with different colors, squamous cell carcinoma(SCC) shows targetoid follicular openings and amorphous white-yellow keratin, and Bowen’s disease represents dotted glomerular vessels and white-yellow scales. Objectives: To determine the effectiveness of dermoscopic margins in NMSC for reducing the final number of MMS(Mohs microscopic surgery) stages. Methods: The effectiveness of dermoscopy was investigated retrospectively by evaluating difference of the final number of MMS stages, final surgical margins, and recurrences. A total of 134 patients(BCC 88, SCC 30, Bowen’s disease 16) undergoing MMS were divided into two groups, naked eyes and dermocopy, and then analysed each other. Results: The final number of MMS stages and final surgical margins were significantly reduced in BCC patients(p = 0.015, 0.000 respectively), but that was not observed in SCC(0.303, 0.000) and Bowen’s disease(0.305, 0.0000). However, the recurrences(2 of 134 patients) were not shown statistically significance in this study(0.159). Conclusion: The results of this study demonstrate that dermoscopic analysis of NMSC to reducing the final number of NMSC stages is especially significant in BCC than SCC and Bowen’s disease.
