Objective: To investigate the impact of type ll collagen (Cll) in patients with rheumatoid arthritis (RA). Methods: T cells proliferative responses to bovine Cll, were examined in the synovial fluid mo nonuclear cerlls(SFMC) and peripheral blood mononuclear cells (PBMC) by mixed lymphocyte culture. CII reactive T cell lines were generated from the SFMC and PBMC. INF-g, IL-12, interleukin-4 (IL-4) were measured by ELISA in the synovial fluids, sera, the culitre supernatant of PBMC and SFMC stimulated with Cll. Results: The frequency of Cll-reactive T cells were higher in the PBMC of RA than PBMC of osteoarthritis(OA) and in healthy controls(HC). In RA CII reactive T cells were more prevalent in SFMC than in PBMC. The mean level of IFN-g and the ratio of IFN-g/IL4 were significantly higher in the culture supernatants of T cells stimulated with Cll and these difference were more prominent SFMC. Moreover, the levels of IL-12 in the culture supernatant of SFMC and PBMC stimulated with CII were significantly higher than those without CII. T cell responsiveness correlated with the levels of type I cytokines of culture supernatant from RA T cells stimulated with CII. In the CII reactive cell lines, the increased production of IFN-g was consistent with the clonal proliferation. Conclusion: CII-reactive T cells were moer appoarently present in SFMC than that in PBMC and strongly associated with a shift toward Th1 cytokines in the inflamed joints of RA. Our result suggests that a skewing to type 1 cytokines by Cll-reactive T cells may play important role in the chronic inflammatory process of RA.
