Cutaneous meningioma has three pathogenic mechanisms: abnormalities of neural tube closure, arachnoid rests in nerve sheaths, and extension of an intracranial tumor. Lipomatous meningioma is a rare variant of metaplastic meningioma. It has benign clinical behavior and is composed of typical menigothelial cells and lipid-laden cells. We report a case of lipomatous metaplastic meningioma in scalp. A 44-year-old female presented a scalp mass since the last 18 years and right side weakness of 6 months duration. Radiologic imaging demonstrated a massive hypervascular mass involving whole scalp with intracranial extension and another multilobulating cystic mass with internal calcification in left frontoparietal vertex. Histologically, the tumor revealed menigotheiliomatous lesion with extensive lipid-laden cells resembling mature adipocytes. Immunoinstochemical study showed that tumor cells expressed EMA, progesterone receptor, p53, and S-100 protein. The origin of the tumor was not identified because the intracranial lesion was not confirmed histopathologically. Key Words: Meningioma, Scalp 237
