Non-functioning Neuroendocrine Carcinoma of Ectopic Pancreas in Colon

The ectopic pancreas is pancreatic tissue found outside the confines of the main gland and may be found anywhere in abdominal organs. It may cause inflammation, bleeding and obstruction in the gastrointestinal tract but the risk of its malignant transformation is extremely low. A 75-year-old women with senile hypertension was admitted for hematochezia. Colonoscope revealed an huge ulceroinfiltrative lesion encircling colonic lumen but could not pass through the lesion, biopsies of which showed large hemorrhagic necrotic tissue by infarction with focal atypical glandular epithelial cell aggregation, suspicious of malignancy. Abdominopelvic computed tomography revealed about 9 cm sized mass with penetrating serosal layer and local invasion of distal pancreas in splenic flexure of colon. We performed radical left hemicolectomy with distal pancreatectomy. Intraoperatively, the huge mass about 10cm in diameters and encircled whole colonic wall in the splenic flexure of colon invaded distal pancreas, and the lymph node in the mesocolon was swollen. Grossly, on opening of the colon, the mass encircled the lumen completely. The cut surface of tumor revealed infiltrating to the pricolic adipose tissue with involvement of adhired pancreas. The remaining colonic mucosa is unremarkable. Histopathologically, the tumor was suggested poorlydifferentiated neuroendocrine carcinoma originated from pancreatic tissue. We reported one case of non-functioning neuroendocrine carcinoma of ectopic pancreas in colon.
