Carcinoma ex pleomorphic adenoma of salivary gland is an uncommon neoplasm, consisting approximately 3.6% of all salivary gland tumors and 2-5% of all pleomorphic adenomas. The carcinoma which develops in a pleomorphic adenoma is most frequently an adenocarcinoma and second most common is undifferentiated carcinoma. Less frequently the malignancy will be squamous cell carcinoma, mucoepidermoid carcinoma, adenoid cystic carcinoma, polymorphous low grade carcinoma, and myoepithelioma. However, adenosquamous carcinoma arising from pleomorphic adenoma has not been reported yet. We report a case of adenosquamous carcinoma developed in a pleomorphic adenoma. The patient was a 48-year-old male presenting with an ovoid elevated palatal mass for 25 years, and rapid growing left neck mass for the recent 3 months. The palatal mass was about 3x2cm in size, well demarcated, and located in the left hard palate with mucosal ulceration. The incisional biopsy of palatal mass revealed squamous cell carcinoma, but radically resected specimen was composed of adenosquamous carcinoma and benign pleomorphic adenoma. There were extensive hyalinization, dystrophic calcification, and keratinizing squamous cell carcinoma which showed transition to the gland-forming adenocarcinoma component. Benign pleomorphic adenoma was present at the peripheral portion of the tumor. The left neck mass showed metastatic squamous cell carcinoma probably from the adenosquamous carcinoma of hard palate.
