Endoscopic sinus surgery has not been questioned to be effective therapy in the treatment of chronic sinusitis. Improvement of symptomatolgy has been reported in as many as 97.5% of patients followed for up to 4 years. However, failure rates in other series have approached 30%, and these figures can be expected of time. The patients who fail initial surgical therapy present an interesting and potentially difficult management dilemma for the rhinologist, due to their altered sinonasal anatomy and associated mucosal diseas. Revision endoscopic sinus surgery is undertaken in sympotomatic patients who, after surgery, have failed additional medical therapy consisiting of oral antibiotics, topical steroid nasal sprays, course of oral corticosteroids, and, when indicated, allergy therapy. These patients manifest a combination of systemic, mucosal, and acatomic problems associated with their recurrent symptoms of rhinitis and sinusitis.
