Calcifying aponeurotic fibroma(CAF) is a very rare fibroblastic lesion. This lesion is benign but has a high incidence of nondestructive recurrence. It is almost always occurs in children and teenagers with a slight male predisposition. Palms, soles, wrists and ankles are typical sites of involvement. It has been observed at other sites, including the neck, forearm, thigh, popliteal fossa, knee, and soft tissues of the lumbosacral region. It areses near tendons, fascia and aponeuroses. CAF presents as a solitary, small, slowly growing, poorly circumscribed non-tender mass. Microscopically, CAF presents a unique pattern of multiple cellular nodules composed of fibroblasts surrounding central punctate areas of calcium or cartilage. Calcification is present in the centers of the nodules except in lesions removed from infants and very young children. We present herein a case of CAF involving left side of the posterior neck in a 15-month-old girl.
