Two mm Video-Thoracoscopic versus Conventional Video-Thoracoscopic Treatment for

Video-assisted thoracic surgery (VATS) has been the choice of treatment for management of recurrent or persistent primary spontaneous pneumothorax (PSP). This study was designed to evaluate the usability of recently developed 2 mm video-thoracoscope and instruments in VATS for pneumothorax. Between January 1998 and June 1999, 62 consecutive patients undergoing VATS for PSP were analyzed. The number of patients undergoing 2 mm VATS was 30(group I), and conventional VATS was 32(group II). The operating duration, the numbers of used Endo-GIA, the need for analgesics, the duration of the chest tube indwelling, and the rate of recurrence were analyzed. There was no significant difference in time of operation, the numbers of Endo-GIA, and duration of the chest tube. The numbers of patients to need parenteral analgesics (ketoprofen, ampule-100 mg) for the first postoperative day were more frequent in group I (56%) than group II (72%), but there was no significant difference. During the follow-up period (mean 15 months), readmission for recurrent pneumothorax was needed a patient in each group. We conclude that 2 mm video-thoracoscope and instruments can be used also successfully to manage PSP with cosmetically excellent results and promise to add an important dimension to VATS.
