Mediastinal lymphangioma treated using EBUS-TBNA

Lymphangiomas are localized malformations of the lymphatic system that most commonly occur in the head and neck. Approximately 90% of lymphangiomas are detected before the age of 2 years, while less than 1% of all lymphangiomas are confined to the mediastinum. In general, mediastinal lymphangiomas in adulthood are found incidentally and remain difficult to treatment. This study reports the first case of safe, effective treatment of a very large mediastinal lymphangioma using endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) in a 29-year-old man. The mediastinal mass had a maximum transverse diameter of 13.7 cm and extended from the inferior aspect of the thyroid gland down to the carina. EBUS-TBNA was performed under mild conscious sedation, and approximately 700 mL of straw-colored fluid was aspirated. Follow-up chest computed tomography 3 months after treatment showed a marked decrease in the volume of the mediastinal lymphangioma, with no early or delayed complications
