외상성 관절염에서의 고관절 전치환술

초록

Post-traumatic arthritis는 initial hip injury의 severity 와 연관되며 적당한 치료에도 발생한다. 또한 injury후 시간이 경과함에 따라 증가된다. 비록 major hip injury를 초기에 치료할 수 있는 능력이 향상 되었음에도 불구하고 많은 환자들에서 reconstructive surgical procedure가 필요로 한다. Anatomic distoration이나 bone loss가 심한 경우 매우 힘들며 젊은 환자에게는 osteotomy나 fusion을 하는 것이 바람직하다. Severe, untreated fracture-dislocation의 경우 acetabular reconstruction이 필요하다. 이때 grafting 과 acetabular reconstruction with implantation of both femoral and acetabular components 하는 one stage operation 시행한다. 젊은 isolated PTA of hip 환자에서 durability와 활기찬 활동을 위하여 cementless acetabular technique을 하는 것이 좋으며, 30세 이하의 환자에서는 hip fusion을 하는 것이 좋다.
