A rare case of pleomorphic adenoma on upper lip

Purpose: Pleomorphic adenoma is a benign tumor that can occur in the salivary glands. It is most commonly found in the parotid gland or submandibular gland. But rarely, it can also occur in the minor salivary glands. These glands are located throughout the mouth, including the upper lip. We would like to share the experience of a rare case for pleomorphic adenoma on upper lip. Methods: A 61-year-old male patient presented with left upper lip mass that occurred 4 years ago. Mass was circular, hard palpable located on the cutaneous upper lip. On pre-operative CT scan, there was a 1.3cm sized fungating mass involving subcutaneous tissue on left upper lip. There were no significant findings in the cervical lymph nodes. Mass was excised with the protruding skin along the skin tension line and direct primary closure were performed. The size and depth of mass were 1.2 x 1.2cm2 and 1.5cm, respectively. Results: On permanent biopsy, the mass was diagnosed as pleomorphic adenoma. It is presumed that this tumor from the minor salivary gland of the oral mucosa of the upper lip grew outward and presenting cutaneous upper lip. Wound totally healed without complication and there was no recurrence during the 1-year follow-up period. Conclusion: The pleomorphic adenoma is the most common benign tumor of salivary gland. However, it is a rare form of head and neck tumor, and even a rarer form of minor salivary gland tumor. Moreover, many benign tumors can occur in the skin, differential diagnosis is not easy when it occurs on the skin of the upper lip, as in this case. Therefore, if a tumor occurs on the skin around the mouth, pleomorphic adenoma should also be considered in diagnosis
