The effect of preoperative education on patient controlled analgesia for pain control of elderly with total knee arthroplasty

Purpose. The purpose of this study was to investigate the effect of preoperative education on patient controlled analgesia for postoperative pain of elderly with total knee arthroplasty Methods. The subjects for this nonequivalent pretest-posttest design study were 50 elderly patients over than 65 years and undergone total knee arthroplasty. Data were collected at one university hospital in Incheon city. To prevent diffusion of treatment, data for the control group were collected at first, and then data collection for the experimental group were completed. Twenty-five subjects were assigned to the experimental group and control group respectively. The experimental group were provided 15 minutes education program with information booklet about PCA use and pain control. The education program also included explanation and demonstration of PCA pump. Results. The experimental group had higher knowledge level about pain and usage of PCA after surgery than the control group (t=6.38 p<.001, t=-3.96 p<.001). The experimental group had more positive attitudes toward pain and usage of painkiller after surgery than the control ones who received general information before surgery(t=5.96, p<.001). There was no significant difference in usage of painkiller after surgery between the two groups (t=-0.27, p=.786). The experimental group had lower pain score at 8, 24 and 36 hours after surgery than the control ones (t=4.03 p<.001, t=5.21 p<.001, t=3.62 p=.001). The experimental group had higher satisfaction level about usage of PCA than the control ones (t=-8.74, p<.001). Conclusion, It was found that the PCA education reduced postoperative pain and increased their satisfaction about usage of PCA. Therefore, PCA education customized by elderly could be an effective nursing intervention on postoperative pain of elderly with total knee arthroplasty.
