Folliscope analysis of temporal involvement in Korean women with female-pattern hair loss

Background: FPHL affects the central portion of the scalp. In some patients, the temporal area of the scalp also can be involved. But,there are few pre-existing classification systems that evaluate this area. Objectives: To evaluate the degree of temporal involvement in Korean women with FPHL Methods: Total of 27 Korean women with FPHL were enrolled. All subjects were classified according to the BASP classification. We evaluated the temporal involvement and added 1 more specific type T to BASP classification(T1:mild,T2:moderate,T3:severe). Hair density and thickness were measured using folliscope. 4 scalp sites including front,vertex,back and temple were targeted. Results: Forty eight percent of patients with FPHL showed temporal involvement. Among 27 subjects,12 and 1 subjects were classified as T1 and T2 each. There was statistically significant correlation between T grade and temporal hair density(p=0.042). The range of temporal hair density was 105.78±29.66/㎠. The temporal/occipital ratio(T/O) of the total hair density was 0.72±0.16. The T/O and frontal/occipital ratio(F/O) of the total hair density was positively correlated(Spearman correlation r=0.502,p=0.008). The T/O and F/O of the thick hair(>80㎛) density was not correlated(Spearman correlation r=0.057,p=0.778). Conclusion: The results in this study suggest that there is decrease in the temporal hair density compared with the occipital hair density in some patients with FPHL. We suggest adding specific type T to BASP classification.
