초록

ntroduction) Diabetes in children and adolescents has increased significantly. youth with Diabetes has markedly high risk of cardiovascular complications. Carotid Intima Media Thickness (cIMT) and Brachial-ankle pulse wave velocity (baPWV), ankle brachial index are good measurement of arterial changes for adult. We evaluated adolescents with type 2 diabetes for arterial change with above tool and assessed risk factors that affected each outcome. Methods) Retrospective cohort study of 113 adolescents (12-18yrs) with diabetes performed cIMT and/or baPWV at a single center from December 2007 to December 2022 was conducted. Subjects were submitted demographic variables, Blood pressure, laboratory data, cIMT. cIMT and baPWV obtained SDS value according to age and sex. Result) Of 113 participants(M=64, F= 49)studied, The mean age was15.3(±1.7). Mean BaPWV was 935.8(±119.2), mean cIMT was 0.557(±0.058).Mean baPWV-SDS was 0.02(±1.00), mean cIMT-SDS was 0.32(±0.46). PWV was increased with duration of diabetes(β =0.237, p=0.004) and diastolic BP(β =0.512, p0.001) in multiple regression. PWV-SDS was increased with diastolic BP (β=0.468, p0.001) pulse pressure and bmi was increased with cIMT. Dyslipidemia increased cIMT(β =0.205 p= 0.035)and cIMT ?SDS (β=0.194 p=0.046). statistically, cIMT and PWV has no correlation with all patients. But High cIMT SDS (z ≥1)was associated high PWV-SDS. Maternal GDM Conclusion) In this study, Carotid IMT changes earlier than PWV in adolescent with Type2 diabetes. But influencing factors are different. Further study is needed for long term follow up and change in PWV and cIMT. Keywords: T2DM, baPWV, carotid IMT
