The Results of Three Day Fractional Chemotherapy using 5-Fluorouracil and Cisplatin for Metastatic or Recurrent Gastric Cancer

The combination of cisplatin and 5-fluorouracil is the most commonly used regimen for the treatment of gastric cancer, usually comprising cisplatin in bolus on day 1 and 5-fluororuracil in fraction on days 1-5, with an average response rate of 40%. We treated inoperable gastric cancer, disseminated on presentation or recurrent after curative surgery, with 3 day fractional doses of both cisplatin and 5-fluorouracil to evaluate the response rate, toxicity, and survival duration. From July 1996 through July 2000, 32 inoperable gastric cancer patients having measurable lesions were treated with cisplatin at a dose of 25mg/m2 in 15 minute drip on days 1-3, and 5-fluorouracil at a dose of 1,000mg/m2 in 12 hour drip on days 1-3. The treatment was repeated for a median of 5 courses (range 2-16) to give 30% of partial response rate (9 of 30 evaluable patients). Complete responder was absent. The median response duration was 6 months ranging 2.3 months to 28 months. Over the median follow-up period of 27 months, the median survival duration was 9.1 months, with a longer duration for responders and shorter duration for non-responders (20.5 months versus 8.0 months, p<0.01). Throughout a total of 166 cycles of chemotherapy, leukopenia was seen in 21%, and thrombocytopenia in 5%, with no treatment related mortality. The 3 day fractional chemotherapy consisting of cisplatin and 5-fluorouracil seemed not inferior to conventional 5 day chemotherapy with regards to response rate and survival duration.
