Foraminotomy and microdiscectomy
초록
Indication of PMF 1. radiologically confirmed lateral foraminal soft disc with appropriate severe radicular symptoms and signs. 2. Osteophytic root compression with appropriate radicular symptoms and signs. 3. Lateral foraminal soft disc or lateral spur with chronic root compression and corresponding radicular symptomatology nor relieved by appropriate conservative measures. Anatomic landmarks C7 is never bifid and is most prominent. General position of the facet joints can be placed approximately 1.5-2 fingers breadth off the midline Preparation and anesthesia A neutral position of the head and neck is maintained to avoid compression the already-compromised root foramen. Positioning 1. Sitting postion 2. Prone position - Mayfield pin headholders or Horseshoe headrest - Ideally, the neck should be slightly flexed(20o) and angled in the reversetrendelenburg position. - Extreme flexion should avoid.
