Cementless Total Hip Arthroplasty in Young Patients under the age of 30

Introduction: Total hip arthroplasty(THA) has shown excellent clinical results in coxarthrosis patients, however, still controversy in young, actie patients even the development of implant technology. This study was performed about clinical and radiologic outcome of the young patients who got total hip arthroplasty under the age of 30 and the patients had been followed up for more than 10 years. Materials and Methods: This retrospective study was conducted for 45 patients (52 hips) undergone surgery by a single surgeon. This study included 27 male patients and 18 female patients. The mean age of the patients when they got the operation was 27.4 years (range 16 ~ 30) and the mean time to follow-up was 11.8 years (range : 10 ? 19.4 years). The most common diagnosis of primary total hip arthroplasty was avascular necrosis(57.8%, 26 cases) followed by sequelae of developmental dysplasia of the hip and septic arthritis(15.6%, 7 cases), traumatic osteoarthritis in five(11.1%), idiopatic coxarthosis in four(8.9%), juvenile rheumatoid arthritis in three(6.7%). We used cementless acetabular component, cementless femoral component and polyethyline liner or ceramic liner. Clinical outcome was researched by Harrison hip evaluation score and activity level, complication was also evaluated. Results: 6 cases had undergone revision arthroplasty in follow up period. The cause of 4 revision cases was aseptic loosening, bilateral case was revised due to periprosthetic fracture and aseptic loosening. 3 cases had undergone both acetabular and femoral component revision, the bilateral case was revised only in acetabular component. 1 re-revision case in this study was performed 12 years 7 months after first stem revision. First revision of re-revision case was 4 year after primary total hip arthroplasty. Overall survival rate was 88.5% at the final follow up with revision for any reason as the end point, especially polyethylene liner using group was 78.6%. All revision cases
