초록
DORSAL HUMP REMOVAL ·Not only one of the most important and delicate procedure but also more than the simple "Removal of the dorsal Hump". ·Performed without an adequate appreciation of the anatomic and functional relationships of the dorsum -> result in significant morbidity. SURGICAL ANATOMY ·Three osseocartlaginous nasal vault-bony, upper cartilaginous, and lower cartilaginous. ·Bony cault - paired nasal bones and ascending frontal process of the maxilla ·Nasal bones - narrowest and thickest above the canthal level ·Key stone area -Junction of upper lateral cartilages with nasal bones and septum -Widest part of dorsum -Relationship of upper lateral cartilages to septum 1)Important, especially in keystone area, contour of dorsum is T-shaped 2)In rhinoplasty or nasal reconstruction this contour of nasal dorsum must be maintained or reconstructed. ·Skin of the nasal dorsum -Thinner in cephalic area of dorsum and thicker in supratip and tip region. -Straight dorsum 1) Slight underlying convexity of ossocartilaginous framework in cephalic area 2) Conbination of this convexity with varying dorsal skin thickness
