만성 뇌경막하 혈종환자에서 배액량과 뇌의 팽창정도의 예측
초록
목적: Chronic subdural hematoma(CSDH) is a relatively common didease seen in the neurological field and occurs frequently in old age. Most neurodurgeons agree that twist drill or burr hole drainage with closed system is a rational therapeutic approach. However, we concerned how much to drain the hematoma and when to remove the catheter. We suggest the way to decide the proper amount of drainage and the time in order to remove the catheter on the preoperative brain CT in CSDH patiens. 대상: Fifty-nine cases in forty-five patiens (man 35 patients 47 cases, woman 10 patiens 12 cases) who were diagnosed CSDH and had an operation between November 2002 and April 2005 at our hospital were investigatred retrospectively. We measured the parameters (mm) on the pre- and post-operative Brain CT as followed (a; distance between both inner table at the level of frontal horn of lateral ventricle, b; distance between most medial side of caudate nucleus head at the same level of a. c; distanceof midline shifting at the level of 3rdventricle d; the largest thickness of chronic subdural hematoma). We estimated the degree of frontal horn compression and midline shifting by the CSDH, and the relationship to the amount of total drainage (student T-test).
