One health surveillance and characterization of extended-spectrum β-lactamase-producing Escherichia coli in Korea

Extended-spectrum β-lactam antimicrobials have been broadly treated to patient seriously infected by gram-negative bacteria. However, the emergence and rapid spread of extended-spectrum β-lactamase (ESBL)-producing Enterobacteriaceae have threatened global health. In this study, we investigate the prevalence and susceptibility of ESBL-producing Escherichia coli collected in Incheon Metropolitan City, Korea, by using cefotaxime-containing MacConkey agar and CHROMagar ESBL plates. Of a total of 1,713 screening specimens from clinical isolates, livestock feces, agricultural products, and farm soil, 131 ESBL-producing strains were isolated. The prevalence was highest in specimens from livestock feces at 54.0%, followed by clinical isolates, farm soil, and agricultural products. Disk diffusion susceptibility test for twenty-one antimicrobials shows that the most isolates were resistant to cefotaxime, cefazolin, piperacillin, and ampicillin but sensitive to meropenem, ertapenem, tigecycline, imipenem, amikacin and cefoxitin. Together, these results define the characteristics of ESBL-producing Escherichia coli in Korea, but also our approach through one health surveillance system may contribute to establishing of a strategic network for global antimicrobial resistance monitoring.
