The relationship between depression and quality of life in patients with dysphagia-Focusing on Effects of meaning in life-

The aim of this study was to determine the factors that have affects on depression, meaning in life, and quality of life of dysphagia paitents while understanding the relevance between those factors. Data were collected by a structured questionnaire from 90 patients who was diagnosed with disphagia in 8 general and rehabilitation hospitals located in Seoul, Incheon or Gyeonggi province. Data were analyzed by descriptive analysis, t-test, one-way ANOVA, and Pearson's correlation coeffiecients using SPSS Statistics 22.0 program. A multiple regression based on Baron and Kenny(1986) was used to determine the mediating and moderating effects. The results of this study were as follows: The total mean score of quality of life in dysphagia patients was 159.45, and in terms of subscale, duration of meal time, burden and communication showed the lowest score with score of 2.74, 2.76 and 3.23 out of 5 points, respectively. There was statistically significant differences between level of education, age, tube feeding and quality of life in dysphagia patients. There was significant correlation between depression, meaning in life and quality of life in dysphagia patients. In Sobel test, meaning in life had significant full mediatory effect with the relationship of depression and quality of life. In terms of subscale of quality of life, symptom and mental showed significant full mediatory effect. Meaning in life did not have significant moderate effect between the relationship of depression and quality of life. But in subscale of quality of life, burden showed significant moderate effect between the relationship of depression and quality of life. As above these results, it was revealed that meaning in life was a mediating variable between depression and quality of life in dysphagia patients. Thus, to improve quality of life in dysphagia patients, it is necessary for the nurses to apply the intervention to improve meaning in life.
