(Summary) a. Changing concepts & therapeutic strategies i. Old concepts - Rely on mainly manual handling, empirical approach 1. Control of abnormal muscle tone 2. Facilitation of normal movement pattern 3. Facilitation of normal postural reaction 4. Practitioner oriented (mattress oriented) Via NDT, PNF, Rood, Brunnstrom technique,and etc. ii. New concepts - Accept modern neuroscience (science based therapy) 1. Biomechanocal concepts 2. Muscle biology, Brain neuroplasticity 3. Patients oriented (task oriented) 4. Learning, and practice theory, and etc. iii. Hybridization and problems - The process of change can be difficult for the practitioner - A temptation to combine newer methods with those already in use. ㆍIntegration new methods with old at times of major change ㆍReconciliation of competing paradigms is superfi- cially attractive Problems -Conceptual differences between competing paradigms - With old concepts if there is lack of evidential support iv. the most acceptable modulator of stroke rehabilita- tion - experts abouts brain neurophysiology, muscle ph- ysiology, bone and joint biomechanics, cognitive neuroscience
