Carcinosarcoma of the Stomach -A Cae Report-

Carcinosarcomas are rare malignant tumors exhibiting light microscopic features of both epithelial and mesenchymal-like differentiation. Such tumors have been referred to by a wide array of terms such as carcinosarcomas, metaplastic carcinoma, pseudosarcomatous carcinomas, carcinoma with mesenchymal stroma, spindle-cell carcinoma, and sarcomatoid carcinoma. Virtually all organs and anatomic sites known to give rise to more conventional tumors of carcinoma have been documented to habor carcinosarcomas. In the gastrointestinal tract, the esophagus is the most common site, and only rare cases have been described in the stomach. Sarcomatous components of the gastric carcinosarcom have been reposted as spindle cell sarcoma with or without smooth muscle characteristics, in most cases. We report a case of carcinosarcoma of the stomach with chondrosarcomatous differentiation in a 57-year-old male patient. He visited health care center of out hospital with complaint of indigesstion. Fiber gastroscopy revealed a Borrmann type II carcinoma in stomach and radical total gastrectomy was performed. The resected stomach revealed a large tumor measuring 5.0x5.0cm. Histologically, the tumor was predominantly composed of myxochondroid sarcomatoid tumor invading to the perigastric fat with lymphovascular invasion. The margin of tumor showed focal adenocarcinoma area consisted of tubular patterns occupying the parts facing the gastric lumen. There were transitions between the sarcomatous and carcinomatous elements. Immunohistochemical stain for cytokeratin showed positive reaction not only in the sarcomatous area but negative in carcinoma area.
