초록
Background : The cytologic reports of primary urethral adenocarcinoma are rare. This tumor is thought be associated with periurethral glands or metaplasia of the surface urothelial mucosa as a result of a chronic inflammatory insult, such as stricture, diverticulum, infection, or fistula. Case : A 80-year-old woman with long-term indwelling urinary catheter due to atonic areflexia presented with urine leakage. On physical examination, a urethral stricture suspicious for malignant lesion was found. Computed tomography revealed periurethral fluid collection and out pouching cystic lesion in left posterior aspect of urethra, regarded as urethritis with urethra diverticulum. Urine cytology disclosed several clusters of atypical tall columnar cells with basally located hyperchromatic nuclei, prominent nucleoli and vacuolated cytoplasm. She underwent cystoscopy showing two urethral masses and biopsy was performed. The histologic section revealed mucinous well differentiated adenocarcinoma with tall columnar cells in the mucinous background.
