Clinical characteristics of diabetic patients according to fasting C-peptide level

The latest classification of diabetes classifies patients as type 1 or type 2 diabetes, but this classification is not always straight forward. Type 2 diabetes is a consolidation of patients with diverse characteristics. Those with decreased insulin secretion who may need insulin therapy earlier in their course may have different characteristics from the majority of type 2 diabetic patients. In this study, we evaluated the clinical characteristics of patients divided by their fasting C-peptide level. 5411 korean diabetic patients were divided into type 1, type 2A, type 2B according to their fasting C-peptide level. Insulin sensitivity was measured by the insulin tolerance test. Type 1 patients consisted of patients with a C-peptide level of ≤ 0.5 ng/ml, type 2A (type 2 diabetic patients with low C-peptide) with a c-peptide level of >0.5 & <1.1 ng/ml, type 2B (type 2 diabetic patients with normal to high c-peptide) with a c-peptide level of ≥1.1ng/ml.
