RATIONALE During ventilator care, head of bed elevation (HOBE) more than 30 degree, oral care with chlorhexidine and hand wasing would be recommended for prevention of ventilator associdated pneumonia. We develop a new device of continuous monitoring of head of bed elevation to improve the performance of HOBE in bundle care. We also measure the performance of HOBE in realtime monitoring and recording. METHODS The attitde and heading reference sstem using sensors was use to measue HOBE. With digital display, this kit was placed just under the board of bed. We validated our method using a protractor. The device was applied on a patient receiving ventilator care. We measured the realtime data of HOBE for 3 days. RESULTS To validate our method, several pairs of measurement were collected on unused beds at angles ranging from 0 to 90 degrees. The correlation between measurements using AHRS and protractor is within minimal variation along the line of equality. The median value of HOB during ventilator care was less than 30 degree. CONCLUSION Our new method can be used to measur HOBE easily. Educatio for HOBE should be doe in practioner of ICU.
