In vivo 1H MR Spectroscopic Features of Renal Cyst and Renal Cell Carcinoma: Initial Experience

목적:The purpose of this study was to investigate differential features of renal cyst and renal cell carcinoma(RCC) with in vivo 1H MR spectroscopy(MRS). 방법:In vivo 1H MRS was performed in 5 renal cysts and 7 RCCs. MRS was performed with a 1.5T unit using localized proton STEAM sequence. The body coil was used as a receiver coil and no controlled respiration was undertaken during the examinations. The parameters used were: TR = 3000 ms, TE = 30 ms, number of scans = 128, and one NEX. An ROI voxel of 1­-8(1³­-2³)cm³ was placed on center of cyst or RCC. We analyzed the patterns of MR spectrums of renal cyst and RCC by recognizing lipid peak(1.3ppm) and protein peaks(2-4ppm). We measured area of lipid peak, protein peaks and water peak(4.7ppm), and calculated peak area ratio of lipid/water, proteins/water and lipid/proteins. Particular attention was given to identify differences in lipid/protein peak ratio between renal cyst and RCC. 결과:Lipid peaks were the most prominent peaks in 4 of 5 renal cysts and 4 of 7 RCCs. Ptein peak was the most prominent peak in 1 RCC. There were no peak in 1 cyst and 2 RCCs. Lipid/water peak area ratio of renal cyst and RCC were 0.00002352±0.00005049(cyst) and 0.00000029±0.00000025(RCC). Protein/water ratio were 0.00000336±0.00000653(cyst) and 0.00000066±0.00000044(RCC). Lipid/protein ratio were 3.43±3.78(cyst) and 0.47±0.21(RCC). 결론: The result of this study showed that there were some differences in the spectral patterns of 1H MRS in renal cyst and RCC, and renal cyst contained larger amount of lipid and protein than RCC. Knowledge of these, spectral differences may provide scientific ground for 1H MRS as new non-invasive diagnostic tool of renal cyst and RCC.
