초록

In the past studies, it was reported that vertigo was accompanied by sudden deafness patients from 25% to 47% and it increased to 56% when electronystagmography was used. They also reported that when sudden deafness patients showed vertigo symptom, the initial hearing losses were severe and their prognoses were poor. But they divided the sudden deafness patients simply by whether vertigo existed, not considering the extent of hearing loss. So we compared the prognosis between the sudden deafness patients showing the same level of hearing loss. We sampled 157 sudden deafness patients (Sep. 1999？Mar. 2002). Of these, 18 patients with acute vertigo (group A) and 18 control patients without vertigo (group B) having the same level of mean air conduction (MAC) as group A were selected, and 20 randomized control patients with vestibular loss only (group C, gain of 0.04Hz in SHA test) who were under the same vestibular rehabilitation treatment and had the same mean ages (46 years old) and follow up periods (6 months) as group A and B were recruited. Group A showed severer initial hearing loss (n=18, MAC 85dB) than that of all sudden deafness patients (n=157, MAC 45dB). They also showed frequent flat form of audiogram (72%, 13/18), poor response to treatment regardless of the initiation time of treatment and slow recovery in vestibular function. In sudden deafness patients, objectively proven acute vertigo symptom is a prognostic factor of hearing and vestibular function.
