Influencing factors on using day care services and features of dementia elderly: retrospective study

Purpose. Using dementia daily care center on regular basis is recommended to delay decline of cognition and daily activity and to reduce the care burden of care givers. The purpose of this retrospective study was to investigate which variables affect the total using duration of daily care center and characteristics of dementia elderly by the time of stop using service. Methods. The study was conducted using a retrospective chart review method and data were collected from 3 dementia day care centers in Incheon, Korea. Subjects were 176 dementia elderly who ended up using the center because of various reasons such as worsening of problematic behaviors, difficulty of ambulation, death etc. Recruited subject data were 183 but 7 cases were deleted from final analysis due to missing data. Results. The mean age of subjects 80.47±5.42 years, mean using period was 17.97±18.67 months. In average, care giver and family member took care of elderly for 33.20±28.01 months. At beginning, cognition (MMSE-K) score, activity of daily living (ADL) and instrumental activity of daily living (IADS) score were 12.31±5.42, 9.56±2.57, and 24.95±5.62 in average, respectively. By the time of ended up, MMSE-K, ADL and IADL were worse than beginning, 11.29±5.80, 11.29±3.28, and 26.91±5.32 respectively. The main reasons to stop using the center were deterioration of health (40.2%), dementia behaviors (19.9%). The significant influencing factors of using duration of daily care centers were the number of dementia behaviors, ADL, aggressiveness, and rejection measured lastly. All these variables explained 18% of variance of duration of using. Conclusion. According to the results of this study, daily care center need to enhance the program and 작업요법 promoting activity and delaying decline of ADL.
