Effect of nebivolol on lipid and metabolic profiles in Korean patients with hypertension: Result from BENEFIT-KOREA study

Background: Unlike conventional beta blockers, several studies proposed the beneficial effect of nebivolol on lipid and metabolic profiles. However, there is no data so far demonstrating the impact of this new generation beta blocker on lipid and metabolic profiles especially in Asian population. Method: From 3011 enrolled patients in BENEFIT-KOREA study (BEnefits after 24 weeks of NEbivolol administration For essential hypertensIon patients wiTh various co-morbidities and treatment environments in KOREA), 754 patients who undergone blood sampling during the study period were investigated. Baseline lipid and metabolic parameters were compared to 12 week and/or 24 week parameters. Patients who had altered lipid lowering medication during the study period were excluded from the analysis. Result: Mean age of the cohort was 63.5?12.9 years old with male predominance (59.6%). 50.5% of patients were dyslipidemic, 37.4% overweight/obese, and 28.9% diabetic, respectively. Mean dosage of nebivolol was 4.5?1.0 mg, suggesting that overwhelming majority of patients received nebivolol 5 mg (76.9%) as their daily prescription. Blood pressure decreased significantly after 12 weeks and this effect persisted to 24 weeks (141.3/83.2 mmHg at baseline, 130.1/75.7 mmHg at 12 weeks, 129.5/75.1 mmHg at 24 weeks, p<0.0001 for each). Although there was no meaningful change on body mass index (BMI) during the study period (BMI 25.8 kg/m2 at baseline, 25.4 kg/m2 at 12 weeks, 25.2 kg/m2 at 24 weeks, respectively), total cholesterol and LDL cholesterol reduced significantly after 24 weeks. Total cholesterol decreased from 168.4 mg/dl to 151.7 mg/dl with 15.5 mg/dl difference (p<0.0001) and LDL cholesterol decreased from 99.9 mg/dl to 84.1 mg/dl with 22.0 mg/dl difference (p<0.0001). Adverse drug reaction occurred only in 0.3%. Conclusion: Nebivolol is effective in terms of reducing blood pressure in Asian patients with hypertension. Furthermore, nebivolol has beneficial or at least neutral.
