수근관 증후군에서 이학적 검사와 신경전도 검사의 중등도의 상관성

초록

Objective: To evaluate the relationship of electrodiagnostic severity and physical examination (Tinnel test, Phalen’s test, Hand elevation test) in carpal tunnel syndrome (CTS). Method: The twenty five patients (48 hands with CTS) with documented CTS in electrodiagnosis laboratory were included prospectively (5 male, 20 female, averaged age 51.8±9.4 years). We evaluated the clinical severity (especially onset time) with 3 kinds of examination (Tinnel test, Phalen's test, Hand elevation Test (HET) for each hand. The electrodiagnostic severity was classified according to Bland JDP’s classification and compared with the existance of symptom and clinical severity (especially onset time of phalen’s test & hand elevation test) Results: The electrodiagnostic severity was classified into six degrees. 2 hands (2%) were classified into very mild degree, 26 hands (52%) mild degree, 12 hands (24.0%) moderate severe degree, 8 hands (16.0%) severe and very severe degree. Tinel test and HET were positive only in 21 hands (43.8%). The average of onset time of Phalen’s test were 31.6 ± 12.3sec. There were significant correlation (p<0.05) between severity of CTS and the onset time of Phalen’s test. Hand elevation tests showed a tendency (p=0.09) but didn’t reveal significant relationship with electrodiagnostic severity (p>0.05). Conclusion: There has been meaningful relationship between the severity of Phallen’s test and that of the nerve conduction study.
