Peripheral T cell lymphoma developing in Hodgkin's lymphoma

Hodgkin's ;ymphoma (HL) has been linked to an increased risk of second malignant neoplasm (SMN), especially non-Hodgkin's lymphoma and acute nonlymphoblastic leukemia. The mutagenic property of cytotoxic therapy as well as defective immunity have been implicated as playing a major role in the development of SMN in patientpriviously treated for HL. We report a case of a 73-year-old man with HL who developed a second malignancy with in a latent period of 36 month following HL diagnosis. The patient presented initially with bilateral cervical and supraclavicular as well as mediastinal, paraaortic lymphadenopathy and bone marrow involvemnet. He was staged as IVB nodular sclerosing type HL and was 6 cycles of CHOP. After 36 month following completion of therapy a peripheral T cell lymphoma appeared in neck lymph node, no evidence of HL in biopsy specimen. Although institution of a new chemotherapy regimen (COPBLAM-V) was done, an unsatisfactory response with a rapid fatal course was noted.
